I need this… Am I eligible?

You can purchase this plan once you are covered under your employer’s group policy and under sixty (60) years. Your eligible dependents may also be covered under this plan.

 

What does this plan cover?

When will it start?

Coverage will begin once we receive your electronic enrollment form, as well as, the first month’s premium through salary deduction.

How do I pay?

Premium Payments are made by monthly salary deduction. The premiums quoted are valid for a minimum of twelve (12) months. 

How do I sign up?

You may click the “Enroll Now” button to choose your coverage option and complete the enrollment process. If you enroll within three (3) months of receiving the offer, no medical evidence is required.
