For Travel Claims the following documents are required to assist in prompt resolution of your matter :-

• Completed claim form
• Original travel insurance policy booklet with schedule
• Copy of ID
• Quote for replacement of item if loss or stolen
• Invoices for items lost and replace
• Invoices for medical services
• Medical report
• Invoices for additional curtailment flight and hotel

Please note we may require additional documents depending on the claim submitted.
